What to bring
- Your school’s, league’s, or camp’s required physical form (already filled in with health history where you can)
- For athletic and high school physicals, the Pre-participation Physical Evaluation form
- The signed Consent to Treatment of Minor Child form
What to expect
- We’ll review your child’s medical history and immunizations and perform a physical exam to confirm they can safely participate.
- We complete and return any required forms before you leave.
- With your permission, a summary of the visit can be sent to your child’s primary care provider.
Good to know: Calling ahead is strongly recommended so we can confirm a doctor is available when you’d like to come in. Most school, sports, and camp physicals are not covered by insurance — payment by cash, debit, or credit card is due at the time of service.
