Child receiving Back-to-school physical Most camp, back-to-school and sports physicals are not covered by insurance. Payment via cash, debit or credit card is due at the time of service. Bring any required forms including a completed health history.

What to expect at your visit:

  • While no appointment is necessary, it is best to call ahead and verify that one of our  doctors can take you at the time you would like to come in.
  • We will review your medical history and immunizations and perform a simple physical exam to determine if you are able to safely participate in your chosen sport.
  • At the end of your visit, we will send you home with any required medical forms.
  • The summary of your visit can be sent to your primary care provider with your permission.
